Declaration or Oath/ Affirmation made at one of the Home Affairs Enquiry Centres (This form shall be used solely as a reference for oath-taking or as an attachment to the affirmation, applicant shall submit the official statutory declaration along with the application form.)

Specified Form signed by the Chairman/ Vice-chairman of one of the recognised labour unions (This form can only be used for application for Intermediate Trade Tests and alternative formats of documentary proofs issued by labour unions will not be accepted.)
